Care and Wellbeing

Health Vault

Medications, GP details, care plans and emergency contacts, kept securely.

A phone showing the care app, held above a desk.

The problem

The information is never where you need it

In a hospital waiting room, someone always asks what medication is being taken and who the GP is. The answer tends to live in a drawer, on a repeat prescription slip, or in the memory of whoever is not there.

How it works

Health Vault, step by step.

  1. 1

    Add the essentials

Medication list, GP surgery, allergies, care plan and the people to contact. Photographs of letters count.

  2. 2

    Keep it current

Prompts to review after an appointment or a prescription change, so the vault stays accurate rather than becoming a snapshot.

  3. 3

    Share deliberately

Access is granted per person through the trusted circle. Nobody gets the vault by default.

  4. 4

    Reach it when it matters

Emergency information can be made available quickly to approved contacts, so nobody is scrolling through a drawer.
